Rent is Due and My Tenant Needs a Payment Plan: Forms to Help You.

Rent is due and many of our tenants will not be able to pay all or a portion of their rent this month as a result of COVID-19.

The forms below are intended to assist landlords communicate and work with their tenants who are unable to pay all or a portion of rent. Please remember that we are all in this together and while your tenant may agree to a payment plan, that plan is based on an assumption that we are all going to back to work soon. If for any reason we are unable to return to work next week, you may have to re-negotiate the payment plan.

Who should use these forms?

Any landlord who has a tenant that has been financially impacted or experiencing a hardship as a result of COVID-19.

Why use these forms?

To communicate with your tenant who is unable to pay all or a portion of their rent due an emergency or disaster. You may also use this form to request rental assistance from local or state agencies were available.

Why do I need a Statewide Vendor Number?

In the event that the state provides rental assistance, we have requested that those checks be sent directly to the landlords. In order for a landlord to receive payments from a state agency, the landlord must have a statewide vendor number. This allows you, as a vendor, to receive payments from all participating state agencies. This also allows you to receive payments by direct deposit, the state's preferred method of payment. It can take up to thirty days to receive your statewide vendor number.

You can register by completing the Vendor/payee registration form. As soon as we have recorded your registration, we will send you an email (If you provide an email on the registration form) confirming the registration and letting you know your Statewide Vendor Number.

Tenant Rental Relief Form and Declaration

Use this form for tenants who are eligible to receive rental assistance through a local or state agency. You may want your tenant to complete the Resident Statement and Requested Payment Plan along with this form.

Resident Statement and Requested Payment Plan

Use this form for a tenant who is unable to pay all or a portion of rent and is requesting a payment plan. You may want your tenant to complete the Tenant Rental Relief Form and Declaration along with this form.

Resident Rent Invoice

Use this form in place of a 14-Day Notice to Pay or Vacate during the statewide moratorium (though April 17, 2020) to communicate with a tenant who hasn't notified you that they are unable to pay rent.
